Hi, I’m River — a former midwife, paramedic, and public health advocate with over 16 years in healthcare. More than titles, I’m deeply passionate about creating real, honest spaces where families feel seen, supported, and empowered at every stage of their journey.

URBANFAMILIES was built from the ground up — dreamed, created, and self-funded — not only as a resource for parents, but as a living, breathing platform for connection. It is my love letter to families navigating pregnancy, birth, postpartum and early parenthood, and equally a collaborative home for a trusted village of practitioners. Alongside me, doulas, midwives, therapists, bodyworkers, lactation consultants and educators come together to share their skills, collaborate ethically, and offer families truly holistic, joined-up care.

My background spans hospital settings, home births, public health projects and infant feeding advocacy, shaped further by lived experience of motherhood, rebuilding life, and finding strength through creativity and community. That blend of clinical knowledge and real life informs everything we offer through URBANFAMILIES .

 

Over the years, supporting families has also meant welcoming their furry family members — dogs curled beside newborns, cats quietly observing from the corner of the room. For many families, pets are part of the family too, which is why UrbanFamilies also includes Dog and Cat First Aid, recognising that care and preparedness extend beyond humans.

Through URBANFAMILIES and our practitioner village, the intention is simple: to offer support that feels human — honest, practical and full of heart. Because caregiving, for babies, parents and pets, is not meant to be done alone.

Our Village

Our Village is a collective of carefully chosen practitioners who each bring their own expertise, lived experience, and way of caring — but who share the same values at heart. Together, we create a truly 360° holistic support system for families, weaving pregnancy, birth, postpartum, parenthood and childhood into one connected journey. By working collaboratively across disciplines, we offer families continuity, depth and care that doesn’t exist in isolation. This is not a directory of services, but a living village — built on trust, respect and the belief that families thrive best when they are held by many skilled, compassionate hands.

Postpartum Specialist

Khyati Desai-Seltzer 

Khyati Desai-Seltzer is the founder of Vyana Infant Massage LLC, where she supports families through the art and science of infant massage and traditional belly binding practices. Rooted in ancient indigenous wisdom and modern understanding of infant development and postpartum care, her work offers gentle, effective tools to support improved sleep, relief from gas and colic, stronger immunity, healthy digestion, deeper bonding, and overall growth and wellbeing for the mother. As a mother herself, Khyati is passionate about empowering parents to care for mama and her babies with confidence, intention, and connection.  Originally from India and having grown up in the United States, she now lives near Colares with her husband and two daughters.

Infant & Child Development Practitioner 

Sigal Avital

My name is Sigal Avital. For over 25 years, I’ve been working in the fields of movement and healing, supporting babies, children, and adults in developing better movement, posture, body awareness, and overall well-being. My work is inspired by the teachings of Dr. Moshe Feldenkrais and Jeremy Krauss, developer of the Jeremy Krauss Approach (JKA) — a gentle, attentive method focused on early developmental stages in babies, as well as working with children with special needs. Movement has always been my language. I believe that through early, respectful movement and presence, we can support confidence, emotional regulation, and healthy development from the very beginning of life. I love working with parents and families, offering a calm, supportive space, practical tools, and guidance tailored to each child’s unique path.

Physiotherapist & Scar Specialist 

Octavia Hamilton

My passion is helping women rebuild strength, confidence, and resilience in their bodies after birth, surgery, or cancer. With over 18 years’ experience as a physiotherapist specialising in women’s health and cancer rehabilitation, I combine physiotherapy, Pilates, biomechanics, personal training principles, and scar therapy to support recovery from the foundations up. My work focuses on posture, functional movement, scar optimisation, education, and mindset — empowering women to understand their bodies and recover with confidence. Doula & Perinatal Educator

Zohra Thilges

My name is Zohra. I’m a mother to a little boy, currently expecting my second baby, and living in Linda-a-Velha with my husband and family. Becoming a mother showed me how essential support is during pregnancy, birth, and the postpartum period, and how even one supportive presence can make a profound difference. This experience inspired me to train as a Doula and Perinatal Educator. My mission is to support women and parents emotionally and informationally through pregnancy, birth, and early parenthood. I’m especially passionate about mother blessings and creating warm, safe spaces where parents can gather, share, learn, and connect. May your journey into motherhood be met with gentleness, trust, and support.

Emily Fraser

I am a naturopathic nutritionist and yoga teacher with a background in personal training, offering a holistic, evidence-informed approach to health and wellbeing. My work combines nutritional science, naturopathic principles, lifestyle medicine and mindful movement to support sustainable, long-term health. I work collaboratively with clients to explore underlying imbalances contributing to their symptoms, creating personalised and practical plans around nutrition, movement, stress management and daily habits. With a strong focus on education and empowerment, I aim to provide clear guidance and compassionate support, helping clients build resilience, improve energy and develop a deeper understanding of their own health.

Midwife & Lactation Consultant

Lara Olchanetzky Duke

I’m UK Registered Midwife and Lactation Consultant (IBCLC). I live in Cascais and support families across Greater Lisbon. I trained and worked in the UK as a birth centre and community midwife. Since moving to Portugal in 2021, I’ve focused on prenatal education, breastfeeding support, Mother Healing workshops for women who have experienced trauma or loss, and postpartum Closing the Bones rituals. I run a monthly Birth and Breastfeeding Café in Cascais and host first aid courses with my colleague River. My philosophy of care is to guide, nurture and empower women and families through the transition into parenthood, offering safe, compassionate care rooted in evidence, advocacy and love. I offer my services in English, French and Portuguese.

Leah Lissauer

I am a Licensed and Certified Professional Midwife (USA, inactive), Doula, International School Teacher and Mother, offering holistic, microbiome-mindful perinatal education for growing families. With experience in home, birth centre and hospital settings, I bridge timeless birth wisdom with current scientific research to support active, physiologic and mindful birth preparation. My work focuses on comprehensive birth and baby education, breastfeeding, nurturing the infant microbiome and conscious parenting through the fourth trimester and beyond. I believe families deserve personalised, respectful and evidence-based care that empowers confidence and connection. I offer services internationally, online and in person when possible.

Ana Alexandra Figueiredo

Parenting is one of the greatest acts of responsibility and love we take on, and adolescence can be one of its most challenging stages. This is often when unhealed emotions, stress and fear surface, and without awareness we can unintentionally disconnect from our children when they need us most. I believe raising resilient, emotionally intelligent and happy young people requires presence, emotional responsibility and connection rooted in safety and love. I am the mother of a young adult and a teenager, and the founder of O Círculo – Uma Mãe Sozinha Não Chega, the first off-social-media community in Portugal dedicated to supporting mothers of adolescents through this demanding and beautiful phase.

Originally from the United States, my work has been shaped by living in diverse cultures and training in both therapeutic and spiritual modalities. With nearly nine years of experience, I offer trauma-informed craniosacral therapy for women, children and families, supporting emotional regulation, stress relief and trauma recovery. My approach blends bodywork, breathwork and intuitive healing to help clients reconnect with safety and inner stability.
